Hello! While I'm not a therapist or counselor, I am HIPAA Compliant to protect my Trauma Work and Hybrid clients. Thanks for supporting me in my efforts to support them. Prospective clients will need to fill out these (2) HIPAA forms:

You can either:
If you have any questions, feel free to reach out by email.
- fill them out in your browser: right click the above links (where it says Download File), choose Open Link in New Tab, fill, then save them with your initials and the date (MM/DD/YR), and email them to me; OR
- download them: right click the above links (where it says Download File), choose Save Link As (and then a destination, such as your Download folder); you may need Adobe Reader on your computer (if so click here for a free download); then save them with your initials and the date (MM/DD/YR), and email them to me; OR
- just print, sign, and email me photos of the *signed* pages; OR
- print, sign, and snail mail me the *signed* pages (see address on the HIPAA forms themselves).
